Mark Stabile et Jacqueline Greenblatt pr\u00e9conisent cette solution, tandis que Michel Grignon s\u2019y \u00a0oppose.<\/p>\n<p>Puisant \u00e0 des donn\u00e9es canadiennes et de l\u2019OCDE, Stabile et Greenblatt prennent d\u2019abord la mesure du d\u00e9fi que repr\u00e9sente le financement des soins de sant\u00e9. Ils montrent que les co\u00fbts li\u00e9s aux soins de sant\u00e9 ont augment\u00e9 \u00e0 un rythme plus rapide que les revenus. De 1982 \u00e0 ce jour, par exemple, ils ont augment\u00e9 en moyenne de 6,9 p. 100 par ann\u00e9e, alors que le PIB ne progressait que de 5,7 p. 100. Donn\u00e9es \u00e0 l\u2019appui, les auteurs font \u00e9galement voir que les taux d\u2019imposition moyens ont recul\u00e9 durant cette p\u00e9riode. Ces tendances expliquent que les soins de sant\u00e9 occu- pent aujourd\u2019hui la part du lion des d\u00e9penses de programmes des provinces au d\u00e9triment d\u2019autres secteurs de d\u00e9pense prioritaires. Ce d\u00e9s\u00e9quilibre risque de s\u2019accentuer avec le vieillisse- ment de la population, d\u2019o\u00f9 l\u2019importance pour le Canada d\u2019envisager de nouvelles sources de revenu pour continuer de financer de fa\u00e7on \u00e9quitable et durable notre syst\u00e8me de sant\u00e9.<\/p>\n<p>\u00c0 cette fin, les auteurs estiment qu\u2019il faut s\u00e9rieusement envisager d\u2019introduire un m\u00e9canisme de capitalisation pour aider \u00e0 financer les m\u00e9dicaments pour les personnes \u00e2g\u00e9es de 65 ans et plus. Selon eux, l\u2019augmentation acc\u00e9l\u00e9r\u00e9e des d\u00e9penses en m\u00e9dicaments et le sch\u00e9ma d\u2019utilisa- tion des m\u00e9dicaments sur ordonnance chez cette client\u00e8le font que ce type de d\u00e9pense se pr\u00eate bien \u00e0 un tel mode de financement. Pour illustrer les co\u00fbts de transition, ils pr\u00e9sentent, \u00e0 par- tir de l\u2019exemple du r\u00e9gime de m\u00e9dicaments de l\u2019Ontario, des simulations permettant d\u2019\u00e9valuer selon diff\u00e9rents sc\u00e9narios la part du PIB n\u00e9cessaire au financement du programme sur une p\u00e9- riode de 50 ans. Tout en reconnaissant que ce mode de financement comporte des incon- v\u00e9nients, ils soutiennent que les avantages l\u2019emportent sur les d\u00e9fauts.<\/p>\n<p>Dans son commentaire, Michel Grignon convient qu\u2019une hausse des d\u00e9penses en m\u00e9dicaments est in\u00e9vitable mais affirme que la capitalisation ne r\u00e9glerait pas le probl\u00e8me de financement que cela soul\u00e8ve. Il examine les trois arguments de fond qui sous-tendent la proposition de Stabile et Greenblatt, et conclut que : 1) la capitalisation n\u2019est pas un moyen efficace de faire face \u00e0 l\u2019augmentation syst\u00e9mique des d\u00e9penses en m\u00e9dicaments ; 2) l\u2019impact financier du vieillisse- ment de la population sur les r\u00e9gimes d\u2019assurance-m\u00e9dicaments sera moins important que ne l\u2019anticipent les auteurs ; et 3) le m\u00e9canisme de financement actuel ne cr\u00e9era pas un probl\u00e8me d\u2019\u00e9quit\u00e9 interg\u00e9n\u00e9rationnelle si l\u2019on tient compte des gains de sant\u00e9 d\u00e9coulant de la recherche- d\u00e9veloppement pharmaceutique.
